To expand the resident's knowledge of the pharmacology and therapeutic use of antimicrobial agents and ability to apply this knowledge in providing pharmaceutical care.
To gain an appreciation for
the role of microbiology testing in the diagnostic and management of infectious diseases.
OBJECTIVE:
Through clinical practice and self-directed study, acquire knowledge of disease states and management of common drug-related issues associated with common conditions encountered in Infectious Diseases
(see Figure 1).
Terminal Competencies
At the end of this rotation, the resident will be able to:
1. Discuss, for the patients assigned for work up, assessment and monitoring, the following:
a) basic pathophysiology of the disease state affecting the patient with respect to epidemiology, cause/etiology, clinical presentation, laboratory indices affected, diagnostic criteria;
b) approach to therapeutic management, pharmacologic and non-pharmacologic;
c) available pharmacologic measures and factors affecting the selection of the optimal measure; and
d) incorporation of pharmacokinetic dosing and monitoring into the therapeutic plan for specific patients;
e) pertinent monitoring parameters, including frequency, acceptable endpoints and
duration of monitoring, for both efficacy and toxicity.
2. Discuss the various therapeutic agents (Figure 2) available for use in managing a drug related response with respect to:
a) efficacy, including spectrum of activity;
b) dosage regimen;
c) bioavailability of oral dosage forms;
d) onset of action;
e) toxicity;
f) convenience/ availability, including Formulary status;
g) drug interactions;
h) cost
3. Actively participate in daily clinical rounds with the Infectious Disease Team
a) able to provide appropriate, timely and concise drug information to the team upon request;
b) able to provide input on the choice of antimicrobial agent based on pharmacokinetics,
toxicity, adverse reactions, drug interactions and availability
4. Give an informal presentation to the Infectious Disease Team on a topic of their choice related to drug therapy.
a) independently utilize the available drug information resources to research the pertinent literature;
b) prepare a concise presentation;
c) effectively and efficiently present the information to the team;
d) answer questions with appropriate terminology, conclusions, recommendations and references.
